As a Licensed Clinical Social Worker (LCSW), a Credentialed Alcoholism and Substance Abuse Counselor (CASAC), and someone with a second Master’s degree in Addiction Studies, I bring advanced training and deep clinical expertise to every evaluation I complete. My background allows me to provide assessments that are not only fully compliant with New York State OASAS standards, but also clinically thorough, fair, and grounded in best practice.

My approach is trauma-informed, non-judgmental, and compassionate. What to expect during your evaluation:

Your OASAS-required Clinical Screening and Assessment includes several key components. Understanding each step can help reduce stress and make the process feel more predictable and manageable.

1. Standardized Screening Tools:

You’ll complete a series of evidence-based alcohol and substance use questionnaires. These tools help determine risk level and any patterns that may require further clinical attention.

2. Toxicology Screening (UDS)

A laboratory-based Urine Drug Screen (UDS) with EtG and a minimum 10-panel test is required. You’ll receive detailed instructions for scheduling your test at an approved lab.

3. Psychosocial Assessment

During our appointment — either in-person or via secure Telehealth — we’ll explore relevant history, behavioral health background, and the circumstances surrounding the incident.

4. Collateral Information

A collateral contact (family member, partner, friend, or employer) provides additional supporting information. This step is required by OASAS to ensure a complete and accurate assessment.

To complete your evaluation in a single session, please submit the following 5–7 business days before your appointment:

NYS OASAS Consent Form (authorization to enter information into the IDS)

Driving Abstract from the DMV

Documentation of the incident (arrest report, ticket, or related paperwork)

Completed questionnaire packet

Collateral contact information

Urine Drug Screen results (EtG + 10-panel minimum from an approved lab)

Copy of your driver’s license

How the evaluation works:

My role is to ensure a thorough, fair, and accurate evaluation while maintaining a supportive, compassionate environment. This is not a punitive process—it is an opportunity to understand your situation clearly and move forward with confidence. The Impaired Driver Clinical Screening and Assessment typically takes 60–90 minutes and includes:

  • Review of the incident

  • Exploration of substance use history

  • Review of psychosocial functioning

  • Integration of collateral information

  • Interpretation of toxicology results

  • OASAS-required scoring and clinical determinations

Once your evaluation is complete, all required information will be submitted to the New York State Impaired Driver System (IDS). If you have selected a written report, it will be delivered within your chosen timeframe and can be provided directly to your attorney upon request.

You will also receive clear recommendations based on OASAS standards and guidance on any next steps, referrals, or resources that may be helpful. My goal is for you to leave this process feeling informed, supported, and fully aware of what comes next.
