As a Licensed Clinical Social Worker (LCSW), a Credentialed Alcoholism and Substance Abuse Counselor (CASAC), and someone with a second Master’s degree in Addiction Studies, I bring advanced training and deep clinical expertise to every evaluation I complete. My background allows me to provide assessments that are not only fully compliant with New York State OASAS standards, but also clinically thorough, fair, and grounded in best practice.

What to expect during your evaluation:
Your OASAS-required Clinical Screening and Assessment includes several key components. Understanding each step can help reduce stress and make the process feel more predictable and manageable.
1. Standardized Screening Tools:
You’ll complete a series of evidence-based alcohol and substance use questionnaires. These tools help determine risk level and any patterns that may require further clinical attention.
2. Toxicology Screening (UDS)
A laboratory-based Urine Drug Screen (UDS) with EtG and a minimum 10-panel test is required. You’ll receive detailed instructions for scheduling your test at an approved lab.
3. Psychosocial Assessment
During our appointment — either in-person or via secure Telehealth — we’ll explore relevant history, behavioral health background, and the circumstances surrounding the incident.
4. Collateral Information
A collateral contact (family member, partner, friend, or employer) provides additional supporting information. This step is required by OASAS to ensure a complete and accurate assessment.

To complete your evaluation in a single session, please submit the following 5–7 business days before your appointment:
✓ NYS OASAS Consent Form (authorization to enter information into the IDS)
✓ Driving Abstract from the DMV
✓ Documentation of the incident (arrest report, ticket, or related paperwork)
✓ Completed questionnaire packet
✓ Collateral contact information
✓ Urine Drug Screen results (EtG + 10-panel minimum from an approved lab)
✓ Copy of your driver’s license
How the evaluation works:
My role is to ensure a thorough, fair, and accurate evaluation while maintaining a supportive, compassionate environment. This is not a punitive process—it is an opportunity to understand your situation clearly and move forward with confidence. The Impaired Driver Clinical Screening and Assessment typically takes 60–90 minutes and includes:
Review of the incident
Exploration of substance use history
Review of psychosocial functioning
Integration of collateral information
Interpretation of toxicology results
OASAS-required scoring and clinical determinations

DWI Clinical Screening & Assessment - $550
The Standard Evaluation includes the full New York State OASAS-mandated Clinical Screening and Assessment required after a DWI, DWAI, or impaired driving charge. This service includes the complete 60–90 minute clinical interview, standardized screening tools, a full psychosocial assessment, toxicology interpretation, and the required collateral interview. All information is entered into the NYS Impaired Driver System (IDS) as required. This option is ideal for clients who have flexible legal timelines or who do not require a written report immediately. A written clinical report can be added later if needed, and will follow the standard 5–7 business-day turnaround time.

Expedited Evaluation (48 hour Report) - $950
**Most popular for clients with legal deadlines.
The Expedited Evaluation is designed for individuals who need documentation completed quickly for an upcoming court appearance, attorney request, or DMV requirement. This service includes the full OASAS Clinical Screening and Assessment along with a written clinical report delivered within 48 hours of your evaluation appointment. All materials you submit are reviewed on a priority basis, ensuring there are no delays in your legal process. This option is highly recommended when your attorney advises that the court will need evaluation documentation before the next hearing date or negotiation meeting. Scheduling depends on availability, but report delivery is guaranteed within 48 hours once the evaluation session has been completed.

Priority Evaluation (24 hour Report) - $1,250
Best for urgent or high-pressure legal situations.
The Priority Evaluation offers the highest level of support for urgent or high-stakes legal situations. This service includes the complete OASAS Clinical Screening and Assessment and provides a written clinical report within 24 hours of your evaluation appointment. This package includes enhanced communication with your attorney, a DMV/court verification letter, and priority placement in the schedule whenever possible. While appointment availability may vary, clients selecting the Priority Evaluation receive the fastest report turnaround and the highest level of documentation and legal support. This option is ideal for next-day court dates, time-critical plea negotiations, or situations where prompt compliance is essential.

DMV or court verification letter included

Once your evaluation is complete, all required information will be submitted to the New York State Impaired Driver System (IDS). If you have selected a written report, it will be delivered within your chosen timeframe and can be provided directly to your attorney upon request.
